Robotic Total Gastrectomy for Patients with CDH1 Mutation
Having your stomach removed by a robot seems like something out of a science fiction movie. However, for patients with the CDH1 gene mutation, the procedure—called a total gastrectomy—can save their lives.
At the University of Chicago Medicine’s multidisciplinary CDH1 clinic, Kevin Roggin, MD, Professor of Surgery, works with radiologists, breast oncologists, genetic risk counselors, nutritionists and psychiatrists to treat patients with CDH1 mutations. In men, having the mutation means living with a 70 percent chance of stomach cancer; in women, it is a 53 percent chance, with an additional increased risk of lobular breast cancer.
An open and closed case
Identifying the mutation early is crucial to patient success. The version of stomach cancer caused by a CDH1 mutation does not form a mass, but instead creates multiple tiny nodules under the surface of the stomach, which are not visible on endoscopy. Therefore, stomach cancer is often detected only after it has progressed to an aggressive, late stage. Identifying patients early involves screening for a familial history of stomach cancer and then working with genetic risk counselors to navigate the ethical implications of genetic testing.
Ultimately, patients may opt to go through with a total gastrectomy, which is the only operation associated with eradicating the risk of this type of cancer. Dr. Roggin is one of few surgeons in the Midwest performing robotic total gastrectomy for CDH1 gene mutation carriers. In fact, he has performed over 15 such procedures for these patients over the past two years with excellent results. All patients had complete curative resections with an average length of stay of six days in the hospital and minimal intraoperative blood loss.
“We can do the procedure safely, and people are in the hospital for a shorter period of time. Nothing is compromised from the standpoint of oncological treatment,” explained Dr. Roggin. Typically, total gastrectomies are performed via an open technique, which can lead to more blood loss, more exposure to bacteria, longer recovery periods, more time in the hospital for the patient and more pain. A robotic technique allows patients to return to their normal lives more quickly, with fewer adverse effects and the same outcomes.

Striking differences
While both robotic and laparoscopic techniques utilize small incisions and long instruments, the two are not the same. In conventional laparoscopic surgery, the procedure is visualized on a television screen. Robotic surgery visualization, however, involves 3D rendering and 10 times magnification. “It’s like the difference between a UHF television and a high-definition LCD screen,” said Dr. Roggin.
At the bedside, Dr. Roggin places all the instruments; then he scrubs and sits in a console eight feet away from the patient, using hand controls and foot pedals to perform the operation. While most current laparoscopic instruments may allow only 4 degrees of freedom, robotic instruments offer 6 to 7 degrees, providing functionality similar to that of the human hand. They also enable tiny wrist movements that facilitate working in tight spaces and allow dissection of tissue with perfect motion stabilization.

Robotics opens new horizons
Becoming a leader in robotic total gastrectomies was not something Dr. Roggin ever imagined would happen in his career. “During my training, technologies like laparoscopic surgery were just becoming more popular, but most cancer operations I did were still open,” he said.
Working with departmental colleagues, he did more laparoscopic work until Jeffrey B. Matthews, MD, Chair of the Department of Surgery, and Mitchell C. Posner, MD, Chief of General Surgery and Surgical Oncology, approached him in 2017 with the idea of doing robotic surgery as a way to better serve our patients.
“It was a huge undertaking,” Dr. Roggin remembered. “These were complex operations utilizing a technique that was new to me. But I did the same thing I tell my residents to do when I teach: I read everything I could and spent 100+ hours in our lab on the console, perfecting my technique.” He even practiced robotic suturing on his children’s dolls, after using up castaway tubes from the simulation lab.
Pushing the boundaries
“It has been really refreshing to learn something new,” said Dr. Roggin. “When I was training, I wanted to use my hands and my mind to help people and affect their lives in a positive way. I expected to be doing these big, open, hard cases. It was not until I saw colleagues around the country performing robotic surgeries that I realized this could be me,” he said.
“Even now, I am constantly watching my own and other robotic videos, talking to colleagues and continually learning more,” he said. “It has almost been like a second career—I never expected to have that. I am thankful that I work for leaders who encouraged me, supported me and made sure I was trained the right way.
“Our department could remain at status quo and lean on our reputation, but our leadership encourages us to push the boundaries in a way that minimizes the risk to our patients. Even though I was taking time away from clinical productivity and teaching to learn robotics, they supported me in learning this because it was important for our patients long-term,” said Dr. Roggin.

But can you eat?
One question remains: Can you live without your stomach? While it may seem impossible to the uninitiated, you certainly can, according to Dr. Roggin. “You can still get the proper nutrients to grow and thrive; you just have to change the way that you eat.” Instead of three large meals a day, recovering patients eat eight to ten smaller meals more frequently and keep track of their diet so they learn what works for them. This is not just living, but thriving, cancer-free.
