Access a Community Health Worker

Accessing a community health worker is for patients who were randomized to the C4P group. Once a patient’s unmet needs are identified, our C4P community health worker (CHW) meets with the patient to help address these needs. This may include connecting the patient to local social service organizations, coordinating transportation, navigating complex government and health care systems or integrating the patient more deeply within the C4P clinical team and their communities. The CHW is available to meet with C4P patients and their families over the phone, in the clinic, or in the community where they live, play, and learn.


The C4P CHW helps patients access a number of resources, including the following:

  • Caregiver assistance 
  • Housing programs 
  • Transportation 
  • Food assistance programs
  • Legal aid
  • Utilities assistance programs
