 If you are currently receiving dental care from the UCM Dental Study Clinic and would like to request an appointment or change an appointment date or time, please fill out the fields below. If you want to cancel an appointment, please call the main dental clinic line at (773) 702-8617 for assistance. We will respond to all requests within 1-2 business days.

We will try our best to give you your first option for your appointment. If none of your options are available, we will contact you and try to find the best date and time for you to come to the dental clinic.
