Study Details

My Diabetes, My Community is telephone-based intervention for adults over 60 years old with type 2 diabetes.  The overall goal of MDMC is to test different programs that help older adults with diabetes manage their care goals.  All study visits take place over the phone, in the comfort of your home, and the surveys can be taken over the phone or online.

If you are eligible to participate, you will be randomly assigned to one of three programs we are testing.  The study lasts about one year.  You will complete one call per month (12 calls in total) and each call will take about 20 minutes. You will also take three surveys, one when you first sign up, one after 6 months, and one at the end of the study.  Each survey will take about 25 minutes to complete.

If you are over 60 with type 2 diabetes, join the study and help test our programs.  It may reveal the best way to provide personalized care for older adults with diabetes.

Who Can Participate?

If you are interested in participating: you must be a patient at a University of Chicago clinic or hospital before enrolling in the study.  This is not part of the research.  To join the study, you must also:

  • Be 60 years old or older
  • Have type 2 diabetes
  • Live in the Chicago metropolitan area
  • Have access to a cell phone with internet access OR
  • Have access to a personal email address
